I am also running rev D of this card. I have only found it runs on hardly any distos. list goes as follows.
sabayonlinux - would not load device gets picked up incorrectly wont load in ndiswrapper
dreamlinux - as above will not work at all
samlinux(pclinOS) - works ok with inbuilt software
mepis 6.5 - works great

I hope the peeps that have had trouble in the past have since found out about madwifi drivers because I've used this card with Slackware, Slax, Phlak, Xandros, Kubuntu, and even Knoppix and it works just super. It's very fast and has a solid, stable connection even under adverse conditions ie: many walls in between it and router. Many distros autodetect and setup right from boot 1. There is no need for ndiswrapper or any other jumping thru hoops. Just get madwifi drivers, from subversion (formerly CVS) if you want cutting edge support or simply what is generally available for your kernel or build easily from source. It's a no-brainer even from source and works really well. Highly recommended.

Jimmy
